Parents/Guardians Must Show ID and be listed in Aeries

When mid-day student pick up is necessary, please note that, for the safety of our Comets, the adult picking up must be listed as an Emergency Contact in the student Aeries Parent Portal and be prepared to provide a form of picture ID. This may take an extra minute or two and we appreciate your patience. Students will only be released directly to a verified adult. We will not release students to meet a vehicle.

 

If mid-day medical appointments are necessary, please allow time and consider your child's schedule. It takes some time for us to locate a student on the playground or in the lunchroom. We may need you to wait until the end of the recess. We are one, BIG, happy family at Cooper and appreciate your support.

Durable Medical Equipment - Casts, crutches, scooters, splints, etc.

  Cooper supports students with casts and other devices by following the guidelines on the students medical note. Please look below so you have the  information about what to ask from the doctor's when receiving medical notes for injuries. If you have any questions, please reach out to our front office. We are happy to help!


Vacaville Unified School District Administrative Regulations are what govern our response to student injuries.AR 5142 (e) reads as follows:


Durable Medical Equipment

A student returning or entering school with Durable Medical Equipment (crutches, wheelchair, canes, splints, etc) must present documentation from a physician with the following information:

 

1. That the use of the DME has been authorized by the physician.

2. The diagnosis and duration of the use of the DME.

3. That the student has received training in the use of the DME.

4. Anticipated length of time the student will be excused from PE and/or recess.

 

The student will not be allowed to attend classes or school events until this documentation is received. In addition, students returning to school after surgery, hospitalization, receiving a cast, stitches, or orthopedic appliances require a doctor’s note which states the following:

 

1. The date the student is cleared to return to school.

2. Any restrictions to school participation 3. Date of follow-up appointment.

 

While your student may be excused from recess activities, we will work with you on a plan for your child to access interaction with peers and be safe considering their restrictions.

 

Independent Study

If you find that you must take your child out of school and will be gone for 1 or more consecutive school days, please contact Chantal Little, the Attendance Secretary, to discuss the process for applying.

Independent Study Procedures

  • 10 days prior to the first day a student will be gone for Short Term Independent Study; parents must fill out a form from the office, requesting the Short Term Independent Study packet, indicating the dates the student will not be in school and the reason for the absence.
  • Parents complete the Voluntary Independent Study Agreement Form and obtain the required signatures (student and parent). Return the form to the attendance secretary in front office.
  • The attendance secretary will review the form and follow up with the Administration for approval or denial. Independent Study requests will NOT be approved during District and State Assessments usually held in May.
  • Upon approval of the Independent Study, the teacher will give the student assignments prior to the first day listed on the Independent Study Contract to be completed.
  • All assignments are due upon the student’s return to school. In order to receive full credit all work assigned must be completed. Partial credit will be given for incomplete contracts and the parent will be notified.
